Priority Care Rehab Staffing is hiring licensed Physical Therapists (PT) for a per-diem opportunity in a skilled nursing facility in New Paltz, NY. You’ll work with residents who need skilled physical therapy to improve or maintain mobility, balance, strength, endurance, and overall functional ability.
What You'll Do
Evaluate residents’ mobility, strength, balance, and functional limitations
Develop individualized physical therapy treatment plans
Provide therapeutic exercise, gait training, transfer training, and mobility interventions
Help residents improve safety and independence with walking and daily movement
Monitor progress and adjust treatment plans as needed
Maintain accurate clinical documentation
Collaborate with nursing, rehabilitation, and other members of the interdisciplinary care team
What We’re Looking For
Active New York State Physical Therapist License and Registration
Graduate of an accredited Physical Therapy program
SNF or sub-acute rehabilitation experience preferred
Strong clinical judgment, documentation, and communication skills
Authorized to work in the United States

Setting: Skilled Nursing Facility
Schedule: Per-diem
Employment: Independent Contractor (1099)
Weekly Direct Deposit: hourly rate is based on relevant work experience
